Some moderation of the current protocol was published by Philip and Saigal (2004):
"... we suggest that clamping should be delayed a minimum of 30 to 45
seconds in preterm infants (after which time the fetoplacental circulation
probably has stopped), which will allow a partial transfusion and may reduce
the severity of RDS [respiratory distress syndrome]. In addition, it may minimize
the need for blood pressure support, improve iron stores, maintain Hct at a
higher level, and decrease the need for subsequent blood transfusions. As
neonatologists, it seems appropriate to start a campaign to say to the person
delivering the baby (obstetrician or midwife), "WAIT A MINUTE" before clamping
the cord ..."
